Cost/InsurancesSMPT Physical Therapy in Brooklyn, NY
Cost/Insurances

Physical Therapy is a covered benefit under most insurance plans.  As a courtesy to our patients, insurance is billed directly to the carrier.  Benefits, deductibles and co-payments vary greatly from plan to plan and will be discussed on an individual basis.  We encourage patients to contact their insurance carriers personally to confirm their benefit information.

Insurance verification will be performed prior to your first scheduled appointment. The following insurances are accepted by SMPT:

Medicare, 1199,  AARP medicare,  Aetna  (as of 5/1/2017), Cigna, United Healthcare ( except Oxford), United Healthcare Community Plan (Americhoice), Empire BCBS, and AARP.

SMPT do not accept personal injury, worker’s compensation, and no fault cases.

At the time of your first appointment, you must bring all applicable insurance cards, forms, and claim numbers necessary for insurance billing. Please be aware that information obtained from verification of benefits is not always a guarantee of payment. Physical therapy claims will be submitted by SMPT on behalf of the patient. However the patient is responsible for payment of services rendered should physical therapy claims are denied by insurance carrier.

Any change in insurance coverage, employer, or home address must be reported to the office staff.

Routine questions about costs or payment plans can be directed to our front desk personnel.

SMPT will verify your physical therapy insurance benefits prior to your first appointment. Please note that the faster we receive your insurance information, the quicker we can inform you of your benefits. In some cases, it is necessary for us to ask for your assistance in obtaining information from your insurance carrier.

SECONDARY INSURANCE: If you have a secondary insurance, we will then file with that insurance provided we have been given all required information.

NO SECONDARY INSURANCE: If there is no secondary insurance to file, we will transfer the balance to your responsibility and you will receive a statement sometime during the first week of the month. (Statements are sent out on a monthly basis until the account is paid in full.)

CO-PAY: If you have a co-pay, that amount is due at the time of service. If you have coinsurance (i.e. 20% due from you and 80% due from your insurance), we would like you to make small incremental payments each visit.

We extend discounts if you are undergoing Financial Hardship situations.

If you have no insurance or if SMPT does not accept your insurance: Please call SMPT at: 718-996-1854 for our private pay rates and our front desk personnel will gladly answer all your questions.
